How to Prepare for your First Visit:

• Please print and fill out the NEW PATIENT forms as complete as possible BEFORE your first visit.
• Bring your insurance card (if applicable).
• Make sure you do not brush your tongue the day of your appointment.
• Ensure that you are hydrated and have eaten a light meal prior to your treatment.
• Finally, wear comfortable and preferably loose fit clothing.
